How to Apply

Step 1. Determine if you are eligible for the Self Sufficiency Program

Step 2. Obtain the Pre-Application and Personal Reference Form by either downloading it or contacting the Self-Sufficiency Program Manager, James Simmons at jsimmons@hiphousing.org or 650-348-6660 Ext. 347

Step 3. Complete a Pre-Application (applicant completes) and submit the Personal Reference Form to a non-family member who knows your education and career goals such as a Caseworker, Community Service Worker, Employer, Coach, School Counselor, or Therapist

Step 4. The applicant sends in both the Pre-Application Form AND the Personal Reference Form to Tafisi Suafai at jsimmons@hiphousing.org or by fax to 650-348-0284. Important: Both forms must be sent in together.

Step 5. James Simmons, Self-Sufficiency Program Manager contacts the applicant to go over the “Pre-Application Form” and “Personal Reference Form” to determine eligibility and discuss next steps.

Step 6. If the applicant qualifies, James Simmons, Self-Sufficiency Program Manager, will ask the applicant to submit all required documents by specific date to verify eligibility. An intake appointment will then be scheduled for the applicant after all required documents are submitted by the deadline. (Acceptance is not guaranteed and will also be dependent on Program and Housing Scholarship funding)
