Referral Process for Self-Sufficiency Program
(Housing Scholarship)

Step 1) Download referral packet (Pre-Application & Personal Reference Forms)
or email Tafisi Suafai at tsuafai@hiphousing.org

Step 2) Client completes the Pre-Application Form

Step 3) Case worker, employer, therapist, coach or school counselor completes the Personal Reference Form

Step 4) Client sends in both the Pre-Application Form AND the Personal Reference Form to Tafisi Suafai at tsuafai@hiphousing.org. Important: Both forms must be sent in together for Tafisi to call you.

Step 5) Program Assistant calls client to go over the “Pre-Application Form” and “Personal Reference Form” to see client qualifies to apply to the Self-Sufficiency Program.

Step 6) If client qualifies to apply, Program Assistant schedules an intake appointment for client to apply to the Self-Sufficiency Program!
